MEDICAL & DENTAL INSURANCEPiedmont Plastics provides a medical insurance administered by Blue Cross Blue Shield of North Carolina. The medical plan features prescription drug and vision benefits. The dental insurance plan is administered by ACS Benefit Services (a division of Blue Cross Blue Shield). Coverage begins on the first day of the month following 60 days of full-time employment. FLEXIBLE SPENDING ACCOUNTAll employees eligible for benefit coverage on or before January first of each year are eligible to enroll in the Flexible Spending Account (FSA). The FSA allows employees to have pre-tax deductions from their paycheck for certain medical expenses and eligible dependent care expenses including day care and elder care. The maximum deduction allowance per year is $3,000 for medical expenses and $5,000 for dependent/elder care expenses. SHORT-TERM DISABILITY INSURANCEShort term disability insurance (STD) pays a percentage of your salary if you become temporarily disabled, meaning that you are not able to work for a short period of time due to sickness or injury (excluding on-the-job injuries, which are covered by workers compensation insurance). Piedmont Plastics pays 100% for this coverage for employees. Regular, full-time employees are eligible for this coverage after completing 6 months of continuous employment. Long-term disability insurance (LTD) pays you a percentage of your salary once STD is exhausted and you have become permanently unable to work. Piedmont Plastics pays 100% for this coverage for employees. Regular, full-time employees are eligible for this coverage after completing 6 months of continuous employment. | OBSERVED HOLIDAYS
Nine holidays are paid by Piedmont Plastics each year to full-time employees working over 30 hours per week on a regular basis: New Years Day, Easter, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and the day after, Christmas Day and one Employee Appreciation Day. PAID TIME OFF
Paid time off (PTO) is consolidated into one "bank" of PTO which should be utilized for time away from work for 1) take vacation, 2) cover short-term illness, and 3) care for personal commitments. This plan provides employees with the flexibility to use paid time off as needed. With the exception of the short-term disability plan, the PTO plan is the only paid time off plan. All regular full-time employees begin accruing PTO the month after date of hire. Regular, part-time employees are eligible for PTO on a prorated basis. The established PTO year is the calendar year beginning January 1st and ending December 31st. VOLUNTARY SUPPLEMENTAL LIFE INSURANCEAdditional life insurance for employees, spouses and dependents is available as an optional benefit. This is term life insurance coverage and is guaranteed issue, if the employee enrolls during his/her initial enrollment period. The employee can choose life insurance amounts in $10,000 increments up to $100,000 in additional coverage for him or herself, and can purchase up to 50% of their own coverage for the spouse (up to $30,000 guaranteed issue) and $10,000 each for eligible dependent children. Premiums are taken through payroll deduction. Coverage begins the first of the month following 60 days of employment. 401(k) PROFIT SHARING PLAN
Employees may contribute to a tax deferred retirement savings to the Piedmont Plastics 401(k) Profit Sharing Plan on the enrollment date following 90-days of full time employment. Enrollment is automatic and will occur on February 1, May 1, August 1 and November 1 of each year. The employee is eligible for a discretionary employer matching contribution on the enrollment date following the employees' first anniversary. The discretionary employer match is announced each year. An employee becomes "vested" in the Company Match at a rate of 20% each year starting on the employees' 2nd anniversary. Complete vesting (100%) occurs after six (6) full years of employment. Our plan currently resides with Diversified Investment Advisors. EDUCATIONAL ASSISTANCEPiedmont Plastics will reimburse tuition for courses taken that are job related and pre-approved, after completion on the following schedule: Grade A = 100%; B = 100%; Grade C = 50%; Pass = 75%; D or Below = 0%. Maximum amount of benefit paid per year will be based on years of service: 1-3 years = $2,000; 3-5 years = $3,500; 5 plus years = $4,250. EMPLOYEE ASSISTANCE PROGRAM
